IMPORTANT INFORMATION TO REMEMBER:

• REGISTRATION DEADLINES

• Your Registration/Medical Release Forms must be turned into the BSU
  office by FEBRUARY 21st to receive the early-bird student rate of $40.
• All Registration/Medical Release Forms must be turned into the BSU
  office by MARCH 4th!
• No registrations will be accepted after MARCH 4th.

• PAYMENT

• Payments may be made by cash or check, but please do not send cash in the mail. All checks are to be made payable to YOUR CHURCH and
  then the church will make a payment to Mizzou BSU Ministries.

• CHURCH CONTACT PERSON FORM

• Only one contact person form needs to be submitted for each church
  group that is attending VISION. The contact person, in most cases, is the youth leader who will accompany the students to the retreat.

• REGISTRATION FORMS

• Every student and leader must complete a Registration Form and and Medical Release Form. Incomplete forms will not be accepted.
